• Offer is exclusive to referrer, who has received a direct text message from us and is an existing customer of Fidelity Hearing Centre Pty Ltd (ABN: 49 605 143 253)
• Referee must not be a previous or existing customer of Fidelity Hearing Centre and must be over the age of 26 years of age.
• Referee must mention this promotion at the initial appointment and the referrer’s name.
• Referee must attend an eligible appointment and is liable for the appointment costs associated.  Appointments may be covered by the Australian Government Hearing Services Program, where referee is eligible.
• Eligible appointments include audiological assessments or hearing aid reviews.
• Gift card is of $20 value and will be valid at SUPA IGA Laidley.  Upon meeting above conditions, a $20 gift card will be sent to both the referee and referrer.
